Most writers do their designs on paper first. Back in the day you'd have a whole black book that you filled with sketches to show other writers when you met but I doubt many still bother. I usually just have my letters outlined on a scrappy bit of paper before I hit the wall but the fill is generally determined on the day by what colour paint I have left/how lazy I'm being

Not the best pic as it's 4 photos nailed together so it makes the piece look wonky and disjointed in places but you get the idea! And yes, the ninja is copied from a video game!
