A scalpel.
A good tip is to scoop out as much flesh as possible from the area you're going to work on.... so it's just the tough skin (approx 5-10mm) you're cutting through.

If I was trick or treating, I would not knock on the door of someone who is that handy with a blade...
I might have to carve a few pumpkins next year. The sad thing is that in my street, if I left it on the doorstep, someone would either smash it up or nick it...
